Welcome to Grace and Guidance,
Aging Life Care Advisors, LLC
Our mission at Grace and Guidance Advisors, LLC is to serve as an advocate for seniors and their families. We offer an array of supportive services that help you and your loved ones cope with complicated medical, emotional, legal, and financial situations. We are your trusted partner in navigating these challenges.
Our Services
We conduct a comprehensive biopsychosocial and environmental assessment using medical, functional, safety, legal, and social tools. A Care Plan is tailored to each individual’s situation and modified with the client and family as circumstances change. Independence is always encouraged, and safety and security are addressed.
Acting as strong and effective advocates for clients and their families, we promote their wishes with health care and other providers, ensuring their needs are addressed.
We offer crisis intervention when it is needed, helping clients navigate through emergency departments and hospitalizations, rehab stays, and ensuring that adequate care is available to the client. For families that live at a distance, this can be a much-needed 24/7 emergency contact. We accompany a client to medical appointments when preferred.
We help families and clients evaluate and select appropriate levels of residential options based on their needs and finances.
We utilize our vast network of vetted professionals in the areas of financial, legal, medical, home health, placement services, and advocacy.
Services may include reviewing or overseeing bill paying or consulting with a client’s accountant or Power of Attorney. We provide information on Federal and state entitlements, connecting families to local programs. Additionally, we assist with insurance claims, concerns, and applications.
